Hi Caroline,
It is difficult to understand your questions without any use of punctuation or sentences, so I will try my best to tweak out your symptoms and provide some information.

Your symptoms (currently) are:
pain in your chest, breast, shoulders and between shoulder blades
bloating
tummy problems
heart burn
weight gain ("got fat around my middle")
weakness
tired most of time
feel down (sad?)
heavy periods (2-3 days)
today's period was very heavy with a clot

You have had many tests done on your heart, abdomen and those are normal.
You have experienced pain with heavy periods in the past.

You said you have been having gynecological problems since the age of 18, and now at 40 years of age, you are having the above symptoms. How long have you been experiencing these symptoms listed above?

The primary symptom of menopause or perimenopause is irregular periods, lack of periods or missed periods. The symptoms you currently have could be many different conditions---either physical, emotional or combination of both---and do not themselves indicate menopause or perimenopause.

When you go to your doctor, it is important to have your hormone levels checked, as that will provide the most information. You can read more about this at: New Blood Test (2009) may predict start of menopause.

I hope to hear back from you, to know what your thoughts are about getting your hormone levels checked, but I'm also wondering...if you have been experiencing these symptoms for 6 months-1 year (or longer), what treatments have you tried? Do you feel that your "feeling down" could be depression? Depression can most certainly correlate to other physical symptoms that you've described, whether or not you are entering menopause or not.
