It could help to have your hormone levels checked, if you feel that your symptoms are problematic. (You did not mention if the headaches are severe or long duration, and I am not sure if the spotting is bothersome?).

You may be entering perimenopause, if your menstrual cycles are changing drastically. Otherwise, women's cycles do change throughout the years, and some women may have menstrual symptoms associated with the fluctuating hormones (this is normal) that can include bloating and headaches. You can also have more spotting before or after.

Are you using any type of hormonal contraception? This can also cause spotting in-between periods.
