It's absolutely possible that it came out -- I had the same issue and have spent a lot of time reading from the experience of others. They tend to come out in clots, with cramping, or, sometimes, just to come lose and randomly be peed out. It's not always obvious when it has come out, so you should contact your OBGYN before having sex again! They'll probably do an ultrasound to see whether or not it is still in place.
