Diabetics can certainly paint their toenails. Sometimes, due to poor sugar level control, diabetics will develop diabetic neuropathy which affects nerves and circulation particularly those of the feet. Because of this, some diabetics opt not to paint their toe nails-- since your nails are a good indicator of your health and if they start to turn blue-ish or any other discoloration unfortunately you wouldn't see this with nail polish on. That's why, if you've ever had surgery, they ask you to remove your nail polish.

As far as your health and nail polish, there's no harm done with the polish.
