This also happened to me as well. I am positive that I received this shingle vaccine and just wanted a record of receiving it. I spent half the day on the telephone between my doctor's off and my insurance company. Since I wasn't charged and there is no record of it in my file, then I didn't get billed for the shot. I can't believe that I'm hearing that another person has gone through this besides myself. How many other things didn't get recorded in our chart? Makes me wonder if I need to find another doctor. Plus look at the money this practice lost from not entering injections. Scary
