Precum does not contain sperm, but there may be sperm left over from a previous ejaculation. The sperm can be pushed out along with the precum as it begins to come out.
Sperm cannot get through most fabrics, but if the undergarments are mesh, like lace, or very scanty, such as a thong, there is a possibility.
