Vaginismus is usually caused for vaginal tightness, not an abnormality of the vagina. Vaginismus simply describes the involuntary constriction of the pubococcygeus (PC) muscles. Usually the root of vaginismus is a combination of physical or non-physical triggers that cause the body to anticipate pain.
Non-physical triggers may include: fear, anxiety, trauma, partner issues etc
Physical trigger may include: insufficient lubrication, not enough foreplay which relaxes the muscles, pelvic tumors, cysts, lichen planus, lichen sclerosus, eczema, psoriasis, vaginal prolaps etc

On the other hand, it could be caused by no apparent reason at all. In this case some doctors offer a surgery which can widen the opening of the vagina. I do not recommend having a surgery unless all possible causes are explored.

To start I suggest that you (if you haven't already) get a hypogenic water-based lubricant for both you and him. And do not try penetration unless you are sufficiently aroused. This will help the muscles relax which expands the vaginal opening. You should also make an appointment with a gynecologist to explore other possible causes.
