Schedule an appointment with your internist or dentist. Following a physical evaluation, the physician or dentist  can refer him to a specialist, if necessary. Since the lump has been there for almost one year and has recently caused pain, it needs to be checked soon. There is always the possibility that it may be a early sign of oral cancer.
