Spider veins or varicose veins in chilldren are actually not as rare as we think. They have a tendency to go away on their own, are not painful, and don't come along with any other health conditions.
Since I can't see what your nephews leg looks like, I have to go based on what you're telling me. But if he seems like he's in pain then this may be something else and he should be evaluated by his Pediatrician at his next appointment.
