My daughter is 7 mo old. We believed she had a milk intolerance/allergy. We switched her to soy based powdered formula. She received one bottle before bed on the night of 5/14. On 5/15 she received 2 more bottles. We noted a rash on her legs moving up the trunk of her body. Small red raised pinpoints. I called the pediatrician. Before we made it to our appointment i found she was running a 102.2 fever. Made it to MD. Initially i thought food allergy. MD thought hand/foot/mouth disease. Then she decided not due to labs with wbc 5.8. Eosinophils were elevated indicating an allergic reaction. And mildly anemic which we think is totally unrelated. MD said not bacterial or viral. Suggested alternating ibuprofen and tylenol. Its now 420am on 5/16. I've given benedryl for the rash twice which was believed to be part of the allergy. Fever is at 101.5. And now we've started vomiting green bile. We just gave her pedialyte. She's resting in her crib in a diaper. Further thoughts or suggestions? Im a RN and out of ideas. I thought roseola Maybe... But the rash and fever are concurrent. I'll take any help I can get. Thanks so much!

Like you, I also thought perhaps Roseola. Sometimes with Roseola the fever and rash can be concurrent. Textbook Roseola would be fever first, then once it breaks Rash--- however, that isn't always the case and they could occur simultaneously. One way you can determine if the rash can be caused by Roseola is to see if they blanch when you press down.
What makes me believe that it may be related to a milk allergy is the fact that she is now vomiting, her easinophils are elevated, and babies that are often intolerant to milk-based formula are usually also allergic to soy-based formula. You can talk to your doctor about switching to a hypoallergenic formula. In the meantime, make sure she gets enough fluids-- you can give her pedialyte to make sure she is well hydrated.

Sorry it took me so long to get back. Thanks so much for your help. We ultimately ended up back at the doctors office. And had another set of labs ran. The doctor didn't have a clue what she was doing, so thankfully I do have some medical knowledge on my side. The doctor never could decide what she thought she might have, even after I laid out all the possible options I had found. The doctor didn't even tell us what to watch for, when to worry, or when to call back. My husband and I through all of our research decided that it was Fifths disease. Almost 2 weeks later her texture of skin is finally about normal. The fever lasted 5 days. We found agreeable formula (without the doctor because she knew nothing about formula). So ultimately, we have a happy, healthy baby, and we're switching doctors ;) Again, thanks for your help!
