I have severe chest pains & my right jaw hurts severely also caused by a hiatal hernia. I need to know what options i may have?

This pain is between my Breast top to bottom & i can't deal with that much pain. The pain feels like it's going straight through to my back. It is continuous but throbbing at the same time. My doctor doesn't seem to know just how painful this is. I cannot take the pain & after a couple of hours it goes away but it leaves a soreness in my chest, back & right jaw. I'm desperate because I can't move or the pain intensifies sharply & position helps. It comes without any warning also. I take medication for GERD but this doesn't stop. Please help me. I believe my esophagus is having spasms. I am 58 years old. It began in my late 20's but only comes more frequent with age
