Rash on my knuckles

Periodically, I get a rash- raised patch that itches and burns. It's usually occurs in the same areas: below the knuckles of my right hand, on thumb and middle finger. Also on the back of my right leg halfway up. Why always the same areas? I have used Fluocinolone cream which burns before bed, and in the morning, I'm clear, Then it begins again midday. Any ideas would be appreciated. (I've had this on and off for several months. I've also had it behind my left knee, but that's OK right now.)

Since this rash responded to a steroid cream, it can be an inflammatory process, such as psoriasis.
What does your physician think it might be? Has biopsy of the infected area been suggested?
