90 year old mother w/ advanced osteoperosis is stupping over and needs help putting on clothing and goint to bathroom? What kind of therapy can restore or rehab her to be more independentA?

She spent 12 weeks in rehabilitative nursing home. Needs help walking alone. Lost a vetebrae last year and had a spine fracture. She also is need of catarack surgery. I believe her eyesight has a lot to do with her lack of balance and fear of falling.
