Many women who visit this site have a "chronic illness", while many others do not, and either have a health inquiry or other health "problem". Are you looking for ONLY those women (or men?) who have a specific "chronic illness"?
2. Is this a "Health Communication Survey" or a "Healthy Communication Survey"? I ask because the latter refers more to communication-style (and is the title on the actual survey), whereas the first refers to how we communicate regarding health and wellness issues. Sorry to be picky, but I noticed the discrepancy in your blog and survey title and I became confused!
