Did you take the missed pill as soon as you remembered?
What do you mean you "nearly missed" the 5th pill? Did you take it, or not? Did you take it late, and if so...how many hours late?
If you miss one pill, then take it as soon as you remembered, there is not a substantial increased risk of pregnancy (check with your patient information brochure in your prescription; most of them say that you do not need back-up protection--like a condom--if you just miss one pill and take it soon afterwards).
If you missed two pills in-a-row, your chances of pregnancy are increased, and the more pills you miss, the higher your chances of pregnancy. When you miss a pill, it is important to:
1. Take it as soon as you remember
2. Make up the missed pill following the instructions in your prescription exactly ("doubling-up" on the next day; we can explain this in-depth if you need)
3. Using a condom for the following 7 days as back-up protection
