Many women do not have orgasms with sex, and it has to do with many factors:
- Being physically stimulated in a way that is pleasing to you by your partner
- Your level of intimacy with your current partner
- Your ability to communicate with your current partner regarding your sexual likes and dislikes
- Your mindset/emotions at the time of sex

Orgasms for women are arguably more mental than physical, and it can take a long time to be with a partner to feel comfortable enough to talk about what feels good/what doesn't, guide his/her hand to the right place, and just having a certain "finesse" about the position, touch...it can be a journey of discovery to find out what feels good to you. Having trust and not feeling pressured to have an orgasm are also important factors.

Some women may not be able to have an orgasm for medical conditions, but I have not heard of this being connected with PCOS.
