In all honesty, I would be very surprised if you could find a doctor to prescribe high dose baclofen to "reduce addiction" or for anxiety. I'm sure you know baclofen is used as a muscle relaxer and to reduce spasms caused by illnesses such as multiple sclerosis. Most clinicians are going to be uncomfortable prescribing it in the manner you are suggesting. In addition to the off-label part of it, that type of prescribing can open a doctor up to investigation. Baclofen can cause central nervous system depression and syncope, so large doses can be dangerous.
