Speakingwings - You need to check your insurance plan. Most require a referral from the designated primary care physician, but some insurance plans do not. If you feel this is going to be an issue you may want to see if your plan has nurse case managers that you can work with to address your needs and get support for the referral. If you encounter many, many obstacles you may even want to consider paying out of pocket for an appointment. It's your health that is at stake, afterall, and as Cary noted delays in proper diagnosis and treatment can be harmful.
