At age 17 you are certainly old enough to have your own gynecologist and to not have to discuss your menstrual cycle with a pediatrician. I encourage you to do so, since you have had issues for three years now.
It's difficult to know what is causing your continuing back pain or why your periods are irregular without the benefit of a full medical history and physical exam. Will you make an appointment soon so you can get the answers and help that you need and deserve? Let us know how it goes.
