The fact that it comes and goes sounds like swollen glands.
Most swollen glands or lumps under the skin are not cause for concern. The glands (lymph nodes) on either side of the neck, under the jaw, or behind the ears commonly swell when you have a cold or a sore throat. You do have a lot of other health issues going on, so this might be connected to those issues. Your doctor is not concerned about it, you say. However, if you feel in your gut something is not right, I suggest calling your doctor and ask that this issue is further investigated if nothing else for your peace of mind. Was an ultrasound taken? If not, your doctor (or another doctor if you choose) may suggest that. There is no way to know for sure if it's a thyroid issue unless it is looked to further.
