Two more things to try (even though I'm sure you're tired of that) is apple cider vinegar (apply it to your scalp and leave for a couple of hours. Yes, it'll smell, but it's worth it to try. Do it several times. If it doesn't help, try tea tree oil.
Another thought - have you been tested for an overgrowth of yeast? This can affect the forehead and the scalp very much - a two week course of medication can help. It's worth it to mention this to your doctor. I had a "yeast infection" (overgrowth) of the forehead - little bumps (hundreds) everywhere and constant itching. It cleared right up with medication and I never experienced it again - this was about 20 years ago. The scalp is a continuation of the skin.
