It's a pity your doctor didn't spend more time with you but all the information you need is in the folded up information sheet that comes with your pill packet. You can also find this information on the website of Estelle-35.

You should have used another form of protection for the first 14 days, according to it's website. This is for the first month of taking it only.

I can take 3-4 days for breakthrough bleeding to happen - wait and see what happens in the next few days. In general, you should change your time over the course of a few days but you only changed it by a few hours - and earlier, not later - so that will not affect it's effectiveness. If you don't get your period this week, take a test but as long as you followed the directions carefully, you should be ok. You can read more on the Estelle website.
