Why does my heart rate reach 195 with moderate exercise?

I am 22 years old, 5' 4" 120lbs. I have to say I am not the fittest individual in the world and never have been. I started to go to the gym a few weeks ago. I usually go 2-3 times a week for an hour and a half at a time.

I noticed that my heart rate reaches 170 with very gently exercise and 190+ with moderate exercise after which I stop. Back in high school, during gym sessions my heart rate would reach 200. My resting heart rate is normal, usually between 65 and 75, depending on the time of day.

My heart rate on exertion has really been worrying me. It does not seem to have shown any improvement in the past few weeks either. I would really appreciate some help
