I think all of those anti-biotics are very rough on your tummy.
Try Zithromax its is only 5 days and although its not primarily for what type of infection you have it might do the trick. Also, so your stomach doesnt feel sick and to prevent yeast infections ,eat plenty of yogurt to keep a healthy balance in your digestive track.

I work in the dental field and I know how paralyzing of a fear you have. Theyre are little things you can do to comfort yourself.
1- bring your IPOD or any type of music you can listen to while in the chair.
2- Breathing techniques. Try and breath in through your nose out through your mouth .(breathing through your nose helps with gagging as well)
3- For you anxiety repeat -
"this is UNCOMFORTABLE not DANGEROUS"
"This will pass"
Try and focus any anything in the room you can. A light fixture for example... ask yourself questions about it.. Where type of light is it? Where could you buy one? or even the drapes in the room... what texture is it? I know it sounds far fetched but Sight ..smell and feel of things really takes your mind of the viscous panic cycle your brain gets into.

Lastly, tell the assistant or doctor of your fear and asks if you can take frequent breaks. Tell them you will raise your right hand when you need them to stop.

You will get through this! Youll be glad when you did!!
