I see this thread has been dormant a while. Unfortunately, I'm in the exact same boat, and am wondering if the original poster ever found a diagnosis or solution. I'm in the military, and am still doing the regular PT tests. I've been dealing with the high heart rate during exercise problem for years, but it appears to be getting more difficult to "push through" as I get older. I'm turning 40 soon.

My runs are much slower than they need to be (9+ min/mile), and my HR jumps up into the mid 190's within the first minutes of exercise. With the HR that high, I can't push up the pace any more than I already am.

I did the various stress tests with two different cardiologists back in 2012. Capacity for exercise and recovery rate were good. No real breathing issues (very slight improvement observed with Albuterol), and no arrhythmia. Would love to find any solution to improve this. Running at 165 BPM would be a dream come true for me.
Thanks,
-Joel
