It's always best to tell your doctor everything! Since there are so many things that you're concerned about, it's best to share all of these with your doctor, since he/she is providing you with care.
As for the antibiotic, I'm not 100% clear on what you're saying. But you should never take just one antibiotic. Antibiotics are meant to be taken for a whole "round" (however many are prescribed to you). Taking just one antibiotic will not completely eliminate the bacteria, and can instead make them stronger. This can create the possibility for antibiotic resistance.
