Do you experience the pain during the day? What precipitated the pain two and half months ago? Did you have an injury or accident?

How old are you? What is your height and weight? Do you have any medical conditions, such as diabetes or osteoarthritis?

Have you had any diagnostic imaging studies, such as an MRI of the spine?
