I am 67. I had T/T for Papillary Cancer followed by RAI. Last week I got my blood results where the Tg. level was 13.6 and my TSH was 0.05(It is 5 months since my very lst.Rai after T/T).
I have 2 opinions expressed: 1) The Oncologist says that my Tg. level is high and that it should be ZERO as I do not have the Thyroid anymore. He has asked me to do a PET SCAN followed by another higher dosage of RAI (150mCi), the previous was 100.
The Endocrinologist says that I should give anotgher 6 months and then have a retest of the Tg.Level in the same Lab. If it goes down, well and good, just follow up for trdeatment. If the Tg. level is on the increase, to do the PET SCAN and other realated treatment. In the meantime he told me to carry on taking my Thyroxine(15mcg) to kdeep the TSH suppressed as above.
Could please advice me which of the 2 should is more favourable.
thank you. - Mahendra

Mahendra,
Sorry for the delay.
Our expert replied, "I agree mainly with the oncologist. What was your Tg before? Did they do a post-ablation scan after the first dose?He is right that your Tg is high and that you probably have recurrent cancer. I would do the PET scan, thyrogen-stimulated thyroid scan If you can) and ultrasound and then get the 150 mci of radioactive iodine ablation. Im happy to see you in my LA office if you want"
Hope this helps!
Best,
Danielle.

As you probably know, papillary thyroid cancer is usually a very slow growing cancer. Also it does take some time for the RAI to complete its work. With that said you have to do what your gut feeling is telling you to do. You have to be happy and feel comfortable with the decision. It is up to you.

Hi Anonymous,
Since I am not an expert in the field, I can't answer that. However, I'd be happy to forward your question on to one of our experts and get back to you later this week. Talk soon!
Best,
Danielle.
