can a clamp or band come off of a tubal ligation, and puncture a mans penis head during sex?

I had a tubal ligation done about six years ago. Last night I had sex with my fiancé, and at the end when we were finished we noticed blood. Somehow, he had a perfect vertical slice with a pinhole cut on both sides of the slice. It was a pretty "exact" wound. Is there any chance my clamp or band came off and has traveled southwards' enough to do this to him?? I'm semi freaking out about this.
