Had you ever had painful periods with clots in the past or has it just started happening these past 3 months? Sometimes after a tubal ligation (having your tubes tied), you can experience complications from a condition known as Post Tubal Ligation Syndrome (PTLS). It sounds like it may be unusual to have since you had your tubes tied 10 years ago, but complications can still occur later in life. Symptoms associate with PTLS include heavy and irregular periods, painful periods with severe cramps, and clots from periods.
I would definitely visit your doctor for a thorough exam, especially if you are experiencing severe pain. They will be able to determine exactly what is going on and advise what to do next and how to reduce your period pain.
