ask: what is the latest line of tretment for Trigeminal

Patient is 83 years old and suffering from trigeminal left mandibular since [ast 20 years as is on Carbamazepine dozes from 400mg to 900 mg. right now taking 1200 mg in 3 dividedd doe along with pregabelin 75 mg 2wice a day.

Kindly suggest any other line of treatment as patient is not responding as per guidelines of the line of treatment. assumption of 50% action on the said treatment. no other metabolic disorder seen.
