According to the Centers for Disease Control and Prevention, the failure rate for condoms, in typical use, is 18%. This means that, yes, it is possible for a woman to become pregnant even when a condom is used for protection.
While many people think a woman's cycle comes every 28 days, that is just the average length - a normal cycle is considered anywhere between 21 and 35 days. Your girlfriend may just be late. The only way to know if she is pregnant is if she takes a pregnancy test.
You two may also want to have a discussion about additional forms of birth control if you do not wish to risk a pregnancy at this time.
