Me and my girlfriend are very careful, I always wear a condom and pull out before climax. We also avoid her fertile week but we may have not this past month. She is approximately 3-4 days late for her period, should we be concerned?

Women can be late for many reasons that are not pregnancy related including other hormonal changes, lifestyle changes, stress or just a change in body chemistry. Your use/method of birth control is generally very safe so it's highly unlikely. However, if you'd like reassurance, a pregnancy test would be accurate by now.
