Two Concerns

Hi I have two different matters that's concerning me a lot and I really would like your help in this.
The first concern I have is, well it's embarrassing for me to say this but near my rectum by touch I feel pain and I don't know why. Would this be something my GYN knows? Is this something bad to have pain there that comes and goes? What could this be?
The other matter is I have something on my thigh called keloid and my doctor told me he will remove it. I'm scheduled for such procedure which he's doing it in the office. I know he will numb the area and first thing I'm wondering about from here is would it affect me Godforbid not be able to walk? The biggest concern I have in this is I'm allergic to lidocaine and I found I'm allergic to the epinephrine which gave me bad side affects In the past. I spoke to my doctor about all these already but to be sure by all these is it okay for me to call him and mention that, "are you only using lidocaine alone" without epinephrine? My other concern from here is, I'm afraid I have side effects to lidocaine alone, should I say this to my doctor to? He also mentioned to me carbocane which I'm okay with this because I had it in the past I want to know should I tell him is it okay to only use carbocane?
